Health & Wellness Centre Notice: Please Bring Your Medical Card & Status Card

Long Point First Nation Health and Wellness Centre is currently updating medical files and is inviting community members to assist by bringing the following documents to their next visit:

  • Medical Card (Health Card)

  • Status Card

  • Medical Card and Status Card for your child(ren) (if applicable)

The Health Centre will make copies for their records and ensure your address and phone number are up to date.

Taking a few moments to update your file helps improve service delivery and supports smoother access to care when you need it most. If you are unable to come in right away, you may bring the required documents with you the next time you visit the Health and Wellness Centre.

Having your Medical Card is important, as hospitals may require it to provide services. Your Status Card may also be needed for medication coverage. Keeping these documents current helps prevent delays and ensures you and your family receive timely care.

When you visit the Health Centre, Medical Secretary Monique Buckner will assist you with updating your information.
