Long Point First Nation’s Social Assistance Department would like to inform all Social Assistance beneficiaries that an annual re-evaluation of Social Assistance files is required once per year.
Our records indicate that this re-evaluation has not been completed for several years for some files. As part of this process, beneficiaries may be required to update information such as current address, valid identification, banking information, and child information.
Valid photo identification
Previous 3 months of banking statements
Children’s identification (IDs)
Proof of address, such as:
Hydro bill
Cell phone bill
Driver’s licence with current address
Any valid government-issued document
Beneficiaries will be contacted individually by phone or Messenger with a scheduled appointment date and time. Please attend your appointment as scheduled, unless you have a valid reason you are unable to attend.
Important: Failure to attend your appointment may result in your income security agent holding your cheque or direct deposit until the annual re-evaluation is completed.
The Monthly Declaration Calendar for the year has also been shared. Please note that no payment will be issued if you fail to attend and sign your monthly declaration.
